Approach

The OHS Management System covers induction and training, risk assessments, permits and work authorization, inspections, incident reporting, emergency response and contractor safety. It applies to employees, contractors, vendors and visitors across operating sites.
Hazards are assessed through HIRA and annual Health Risk Assessments, with controls applied according to the hierarchy of elimination, substitution, engineering, administration and personal protective equipment.

Pursue zero work-related fatalities and zero occupational ill health.
Apply critical-risk controls and verify them through audits and inspections.
Strengthen contractor prequalification, monitoring and stop-work authority.
Provide confidential health services, surveillance and accessible hazard reporting.

Performance

Employees recorded one lost-time injury and no recordable injuries. Contractors recorded five lost-time injuries, 22 recordable injuries and three high-consequence injuries. The near-miss frequency rate improved to 1.55 from 1.84. PTVI reported no recordable work-related ill-health cases.

Delivered 70,656 total training hours and provided preventive health check-ups for 17,947 personnel, exceeding internal plans.
